membership policies
At Nurture Functional Medicine, our goal is to provide care that is sustainable, consistent, and respectful of both your time and ours. To keep memberships running smoothly, please review the following policies:
Cancellation Policy
Memberships may be canceled at any time with a 60-day written notice.
All cancellations must be submitted in writing to: support@nurturefunctionalmedicine.co.
Your membership will remain active (and billable) through the end of the 60-day period.
Refunds & Exchanges
Membership fees, consultations, and services are non-refundable and non-transferable.
This policy helps us ensure fairness and maintain reserved availability for all members.
Family Membership Discounts
Families are welcome to join together. We offer a 15% discount on each family member’s membership. There is a limit of 1 discount per member; discounts do not stack (ex: cannot combine early bird & family member discount).
Dependents under the age of 21 may be added at a reduced rate of $75/month (Foundations level).
Please note: family discounts do not apply to early bird / founding member pricing.
Payment & Billing
Membership fees are billed monthly to the card on file.
All payments must be kept current to continue receiving care and access to services.
Communication & Support
Members must use the patient portal for all health-related communication.
Non-urgent messages will be addressed within 3 business days for all patients (Flourish members get priority messaging access).
Urgent or emergency needs should always be directed to 911 or your local emergency department.
What happens if I need more visits than my membership includes?
The Foundations membership includes 3 live visits per year (30 minutes each), 2 asynchronous check-ins, and limited messaging. If you need more support, you have two options:
Upgrade to Flourish at any time — this tier includes up to 6 live visits per year (45 minutes each), unlimited asynchronous check-ins, unlimited messaging with priority response, and complimentary health coaching.
Book an extra visit while remaining in Foundations. Additional visits can be scheduled at the current à la carte rate ($350/hour).
If you consistently need more frequent follow-up, we recommend upgrading to Flourish, which is designed for higher-touch care.
Prescription & Rescheduling Policies
Prescription Refills (for Members)
Continuous Access: As a member, prescription refills are included in your plan — no extra visits are required just for refills. Please keep in mind that refills must be requested with at least 48 hours’ notice in order to avoid gaps in your therapy.
Safe & Effective Care: Refills are provided as long as your membership is current and you are completing required check-ins (Foundations: at least 2 visits per year; Flourish: minimum of 4 visits per year).
Monitoring: Prescriptions may require updated labs at regular intervals to ensure safe dosing. Patients taking medications which require monitoring (including hormone replacement therapy, peptides, and LDN/naltrexone) are required to complete lab work at least twice per year to continue receiving refills.
Adhering to your care plan- including completing required lab work, attending appointments, and requesting refills prior to running out of medication- is the patient’s responsibility. This helps us provide the best care and avoid any delays in your refills.
Appointment Cancellations & Reschedules (for Members)
Courtesy Rescheduling: Members may reschedule appointments at no extra cost as long as notice is given at least 48 hours in advance.
Late Cancellations (<48 hours): These cannot be guaranteed a reschedule due to limited availability.
Missed Appointments / No-Shows: These visits are considered used and will not be credited back.
Rebooking After Missed Appointments: If you wish to reschedule a missed appointment or one canceled with less than 48 hours’ notice, you may be subject to an additional $175 appointment fee to secure a new, 30-minute time slot.
Membership is designed to be flexible, with asynchronous messaging and check-ins built in — so even if life happens, you’ll never lose access to care.
