What is functional medicine?

Functional medicine is a patient-centered approach that looks at the root causes of illness rather than just treating symptoms. It uses advanced lab testing, nutrition, lifestyle, and — when appropriate — prescriptions (such as hormones, thyroid support, or low-dose naltrexone) to restore balance and help the body heal itself.

What ages of patients do you treat?

We currently accept patients ages 2 years and older in Florida & Georgia, or 16 and older in all other states, for functional medicine care.

Can Carly be my primary care provider?

No. Nurture Functional Medicine is a functional medicine specialty practice, not a replacement for your primary care provider (PCP). You should continue to maintain a relationship with a PCP for routine screenings, urgent care, and general medical needs (such as Pap smears or acute issues like infections and injuries). Carly’s role is to partner with you in optimizing your health through functional medicine.

What happens if I need more visits than my membership includes?

The Foundations membership includes 3 live visits per year (30 minutes each), 2 asynchronous check-ins, and limited messaging. If you need more support, you have two options:

  1. Upgrade to Flourish at any time — this tier includes up to 6 live visits per year (45 minutes each), unlimited asynchronous check-ins, unlimited messaging with priority response, and complimentary health coaching.

  2. Book an extra visit while remaining in Foundations. Additional visits can be scheduled at the current à la carte rate ($350/hour).

If you consistently need more frequent follow-up, we recommend upgrading to Flourish, which is designed for higher-touch care.

What states is Carly licensed in?

Currently, medical services are available in Florida, Georgia, Texas, and Ohio. More states will be added based on demand and licensure requirements. If you live outside these states, Carly may be able to provide referrals or recommendations for practitioners in your area- please email us at support@nurturefunctionalmedicine.co.

What is included in messaging support?

  • Foundations: up to 3 secure message exchanges per month, with responses within 2 business days.

  • Flourish: unlimited secure messaging with priority responses within 1 business day.

Messaging is designed for quick clarifications, updates, and follow-up questions. For new or complex concerns, Carly may recommend scheduling a live visit.

How do prescription refills work?

Prescription management (for hormones, thyroid, LDN, etc.) is included in both membership tiers as long as:

  • Your membership is active

  • You stay current with required check-ins and labs

Lab requirements: Patients using hormones, peptides, antifungals, mast cell medications, or LDN/naltrexone must complete labs at least twice per year for safe dosing. It is the patient’s responsibility to stay current on required labs to ensure uninterrupted care. Lab requirements will be discussed before initiating any therapy.

Can I cancel my membership?

Yes. Memberships may be canceled at any time with 60 days’ written notice to support@nurturefunctionalmedicine.co. Services remain active (and billable) through the notice period.

Please note: If you are enrolled at a discounted early bird rate and later choose to rejoin, your membership will resume at the current standard pricing in effect at that time.
